LA Found Bracelet Distribution

Through LA Found, if your loved ones suffers from Alzheimer’s, dementia, autism or any cognitive impairment and is mobile, they may qualify for a free tracking bracelet.

To get a free tracking bracelet (retail value of $375), you must bring the following:

  • Government Issued Identifications for the participant (person in need of the bracelet) and the authorized representative/caregiver;
  • Power of Attorney, conservatorship or healthcare directive; and
  • Participant’s birth certificate if he/she is a minor.

Note: If you don’t have these documents available or if you cannot make it to one of these distribution sites please contact us at LAFound@wdacs.lacounty.gov or at 833-569-7651.
